If you would like to fill out your absentee ballot request form in 2 minutes:
Use a touchscreen (e.g. large smartphone or tablet) because you need to DRAW your signature EXACTLY the way you write it on paper.
A computer mouse is less likely to produce a matching signature.
Your ballot will NOT require notarization if you use Reason #2 (confinement due to illness).
Any other reason WILL require you to get your ballot notarized (like at a bank) after you receive it in the mail.
After submitting your request form, you will get an email with your request form attached. Forward this email to your local election authority.
Call your local election authority to confirm that they received and are processing your ballot request form.
